A copy of the Vendor’s insurance certificate with applicant named as an additional insured is required.
A copy of the Texas Alcoholic Beverage Commission (TABC) Certificate along with applicable permit fees must be submitted prior to issuance of Permit.
Ex. speakers, megaphones, instrument amplifiers etc.

Additional permitting may be needed.
Included ingress, egress, event and handicap parking. Visual depiction must be included in final site maps.
1 restroom per 300 people is recommended. Applicant is responsible for ensuring all trash is cleared from event area at conclusion of event.
A list and map of flags, signs and banners staked or hung on City of Forney property is required. City of Forney reserves the right of refusal.
Based on event components additional staffing may be required at the expense of applicant.

Name the City as additional insured and certificate holder
General Liability insurance coverage must be 1,000,000.00 per occurrence.
If amusements rides are at event: Provide the Vendor's insurance certificate with
applicant named an additional insured.
